Abstract: INTRODUCCIÓN: Las metástasis (MT) son el factor pronóstico (FP) más importante en el cáncer. Cuando las MT son sólo pulmonares, conllevan mejor pronóstico de supervivencia (SV) tras cirugía. Identificar factores pronósticos (ffpp) ayuda a seleccionar los candidatos a cirugía, el tratamiento complementario y la detección de las recaídas. La vía de abordaje (VA) es el aspecto más controvertido del tratamiento quirúrgico. OBJETIVOS: Revisar los ffpp y comparar las vías de abordaje (vvaa) en SV y parámetros postoperatorios. MATERIAL Y MÉTODOS: Estudio descriptivo y analítico de cohortes. Análisis descriptivo y de regresión de variables del tumor primario y las metastasectomías: VA y resección, estancia, complicaciones, secuelas y recaídas. Sensibilidad (S) y valor predictivo positivo (VPp) de las vvaa. Tablas cruzadas, chi cuadrado, Kapplan-Meyer, log-rank y Cox con Hazard Ratio. RESULTADOS: Del año 2001 al 2018, se extirparon 722 metástasis pulmonares (MP) en 378 operaciones realizadas a 310 pacientes. Influyeron significativamente en la SV: diferenciación y estadio del primario; tamaño y número de las MP. La videotoracoscopia (VT) obtuvo mejor SV que la toracotomía (TC). La VT obtuvo menos estancia y complicaciones, y mostró mejor VPp que la TC. CONCLUSIONES: Consideramos el grado de diferenciación de la neoplasia primaria como el factor pronóstico con mayor influencia en la SV. Sugerimos una revisión de los grupos pronósticos clásicos en base a los ffpp que han mostrado influencia en SV. Aconsejamos la VT como VA quirúrgico de elección en las metastasectomías pulmonares.
INTRODUCTION: Metastasis (MT) are the most important prognostic factor (PF) in the cancer. When the MT are only pulmonary they involve a better prognosis for survival (SV) if they are removed. To identify prognostic factors (pff) helps to select candidates for surgery, the adjuvant treatment and the recurrence detections. The method of approach (MA) is the most controversial aspect of the surgical treatment. OBJECTIVES: To review the pff and compare the MA in SV and postoperative parameters. MATERIALS AND METHODS: Descriptive and regression variable analysis. Descriptive and regression analysis of primary tumor and metastasectomies: MA and resection, stay, complications effects, relapses, sensitivity (S) and positive predictive value (pPV) of MA. Cross tables, chi-square, Kaplan-Meyer, log-rank and Cox hazard ratio. RESULTS: From 2001 to 2018, 722 PM were removed in 378 operations carried out to 310 patients. This influenced significantly in the SV: differentiation and stage of the primary one; size and number of the PM. Videotoracoscopy (VT) achieved a better SV than thoracotomy (TC). VT reached less stay and complications, and it showed a better pPV than TC. CONCLUSIONS: We consider the degree of differentiation of the primary neoplasia as the PF with greatest influence in the SV. We suggest a review of the classic prognostic groups based on the pff which have shown influence in the SV. We advise the VT as the surgical MA of choice in pulmonary metastasectomies.
