Resumen: INTRODUCCIÓN: Los estudios observacionales pueden ayudar a conocer la efectividad de los medicamentos en la práctica clínica. OBJETIVO: Analizar la eficacia y seguridad de los inhibidores de la proteasa (telaprevir y boceprevir) en los pacientes con hepatitis C tratados en el SES. MATERIAL Y MÉTODOS: Estudio observacional descriptivo de los pacientes tratados en el SES entre Agosto de 2012 y Junio de 2014, finalizando el seguimiento en Noviembre de 2015. Revisión de las historias clínicas, registros de dispensación de medicamentos y los resultados de laboratorios. Análisis por intención de tratar mediante seguimiento a las 4, 8,12, 24 semanas, finalización terapéutica y a las 24 semanas postratamiento. RESULTADOS: Se incluyeron 103 pacientes 64% hombres. La edad media fue de 52,8 ± 8,5 años. El 54,4% eran genotipo 1b, 36.9% genotipo 1a y 8,7% de genotipo 1 subtipo desconocido. El 72,8% de los pacientes tenían grado de fibrosis hepática > F2. Un 63,2% de los pacientes tratados con telaprevir consiguieron respuestas virales sostenidas y 54,3% de los de boceprevir. Se registraron leucopenias en el 18,4%, neutropenia en el 15,5% de los casos y anemia en el 6,8% todas de grado III/IV. Los abandonos registrados fueron del 9,7%, como motivos destacados la toxicodermia y retinitis severa. CONCLUSIONES: La eficacia de telaprevir y boceprevir encontrada en condiciones reales es mayor a la de la biterapia, aunque inferior a los ensayos clínicos de ambos. Las reacciones adversas fueron más frecuentes y severas que en dichos ensayos clínicos.

OBJECTIVE: Analyze the effectiveness and safety of protease inhibitors (telaprevir and boceprevir) in patients with hepatitis C treated at the SES. MATERIAL AND METHODS: Observational descriptive study of patients treated in the SES between August 2012 and June 2014, ending the follow-up in November 2015. Review of medical records, medicine dispensing records and laboratory results. Intention-to-treat analysis by follow-up at 4, 8, 12, 24 weeks, therapeutic end and post-treatment after 24 weeks. RESULTS: 103 patients were included (64% male). The average age was 52.8 ± 8.5 years. 54.4% were genotype 1b, 36.9% genotype 1a and 8.7% genotype 1 unknown subtype. 72.8% of the patients had grade of liver fibrosis> F2. 63.2% of patients treated with telaprevir and 54.3% treated with boceprevir patients achieved sustained viral responses. Leukopenia appeared in 18.4% of cases, neutropenia in 15.5% and anemia in 6.8% of them, all grade III / IV. 9.7% dropouts were recorded, being toxicoderma and severe retinitis the main causes. CONCLUSIONS: The effectiveness of telaprevir and boceprevir in actual conditions is greater than that proved by bitherapy; nevertheless, effectiveness of both protease inhibitors was lower in real conditions than in trial conditions. Adverse reactions were more frequent and severe in actual conditions.
