Comparison of RANS and LES turbulent flow models in a real stenosis
(Elsevier, 2024) Lopes, Diogo; Puga, Helder; Teixeira, José Carlos; Lima, Rui Alberto; Grilo, José; Dueñas Pamplona, Jorge; Ferrera Llera, Conrado; Universidad de Extremadura. Departamento de Ingeniería Mecánica, Energética y de los Materiales; Universidade do Minho. Portugal; Universidad de Extremadura. This study focuses on the performance of various turbulence models in predicting hemodynamic variables within a patient-specific geometry of the Brachiocephalic trunk exhibiting a severe stenosis. Numerical simulations employing Reynolds-averaged Navier-Stokes (RANS) and Large Eddy Simulation (LES) models were conducted, comparing four RANS with two LES models. Results indicated significant differences in turbulent structures between RANS and LES models, with the k-ω RANS model closely approximating LES throughout the cardiac cycle. The velocity and Turbulent Kinetic Energy (TKE) results were consistent in the stenosis region due to the proximity to the inlet and plug flow conditions. However, notable disparities were observed in bifurcation and outlet regions. Time-Averaged Wall Shear Stress (TAWSS) comparisons revealed that the k-ω-based models provided closer agreement with LES, particularly in atherosclerosis-prone areas. The study highlights the limitations and strengths of each turbulence model, emphasizing the importance of model selection in simulating complex cardiovascular conditions. While RANS models demonstrated computational efficiency, their accuracy varied across regions. The effectiveness of the k-ω model in capturing the intricacies of such a complex flow field suggests its potential as a reliable and accurate tool for simulating physiological conditions with strong curvature and advanced stenosis.

Plasma and urinary concentrations of arachidonic acid-derived eicosanoids are associated with diabetic kidney disease

Preclinical studies indicate that arachidonic acid (AA)-derived eicosanoids contribute to hyperglycemia-induced kidney injury. We aimed to determine whether plasma and/or urinary levels of dihydroxyeicosatrienoic (DHETs) and 20-hydroxyeicosatetraenoic (20-HETE) acids are associated with diabetic kidney disease (DKD). A total of 334 subjects (132 DKD patients and 202 non-diabetic individuals) were studied. Plasma levels of 11,12-DHET, 14,15-DHET and 20-HETE were measured by LC/MS/MS. Urinary 20-HETE concentrations were determined by immunoenzymatic assay. Subjects with normoalbuminuria had larger 20-HETE-to-creatinine urinary ratios (20HETE/Cr) than those with micro and macroalbuminuria (p=0.012). Likewise, participants with eGFR>60 ml/min/1.73 m2 had higher plasma levels of 14,15-DHET (p=0.039) and 20-HETE/Cr ratios (p=0.007). Concentrations of 14,15-DHET, 11,12-DHET and 20-HETE/Cr were significantly lower in DKD patients. Median values for non-diabetic vs. DKD were, respectively, 493 (351.0-691.5) vs. 358 (260.5-522) ng/L, p=3e-5; 262 (183.5356.0) vs. 202 (141.5-278.0) ng/L, p=1e-4 and 5.26 (1.68-11.65) vs. 2.53 (1.01-6.28) ng/mgCr, p=0.010. In addition, 20-HETE/Cr ratios were higher in patients with non-proteinuric DKD than in those with typical DKD (p=0.020). When only individuals with impaired filtration were considered, 14,15-DHET and 11,12-DHET levels were still higher in non-diabetic subjects (p=0.002 and p=0.006, respectively). Our results indicate that AA-derived eicosanoids may play a relevant role in DKD.

Portugal
Currently, the meaning of life is considered as a driver of psychological well-being and significantly relevant to mental health. This positive psychological approach has increasingly made society and the academic community aware of the need to evaluate and investigate these concepts, in order to understand their implications. This study includes a sample of 431 subjects residing in the Federative Republic of Brazil, 100 male and 331 female. Using self report measures, through the application of questionnaires, our research correlated the concepts of meaning in life, psychological well being and mental health, also considering gender, age and personality characteristics. Through data analysis, we were able to understand that the search for meaning in life continues to be predominant in subjects, even though, in a significant sample, we found the statement that there already exists a clear purpose in life. As for psychological well-being, there are positive correlations between its factors and the presence of meaning in life, and it is negatively related to symptoms of depression, anxiety and stress. Regarding the effects of meaning in life and personality on mental health symptoms and psychological well being, it was possible to identify interesting predictors. Another relevant fact is the statistically significant differences between emerging adults and middle-aged adults in terms of the variables analyzed, with no important gender effect being recorded. We conclude that there is evidence that there continues to be a search for the meaning in life and that well-being and mental health are affected by it, therefore, it is up to us to analyze the causes and how these have implications for everyday life of Brazilian citizens. This issue appears as a warning in such a controversial time, post-covid and with the current occurrence of international war conflicts.
