Análisis de los factores asociados a la susceptibilidad a fumar en estudiantes de educación secundaria del proyecto motiva educación secundaria del proyecto motivo EDUCA

Abstract

El índice extendido de susceptibilidad a fumar (IESF) es un marcador de inicio de consumo que permite identificar a los jóvenes que están en riesgo de iniciar el consumo de tabaco. El objetivo de este estudio fue analizar los factores asociados al IESF en estudiantes de 2º-4º de educación secundaria no fumadores. Se realizó un estudio transversal en tres centros educativos de Cáceres. Recogiéndose mediante cuestionario autoadministrado, variables sociodemográficas, consumo de cigarrillos (a nivel personal, familiar y del entorno social), consumo de alcohol y otras sustancias, factores intrapersonales (impulsividad y afectividad) y el IESF que se categorizó en tres niveles. De los 377 participantes (47% chicos, mediana de edad de 15 [14-15]años), 226 (60%) no habían probado los cigarrillos, de ellos, 110 (48,7%) fueron clasificados como susceptible-altamente susceptibles. Los análisis multivariantes mostraron magnitudes de asociación significativa entre consumo familiar, consumir alcohol, tener amigos fumadores con la condición altamente susceptible, con valores ORa: 2,78 (IC95%: 1,17-6,51) p=0,020, ORa: 4,67 (IC95%: 1,74-12,5) p=0,002 y ORa: 3,85 (IC95%: 1,67-9,04) p=0,002 y las dimensiones urgencia y falta de perseverancia de la impulsividad se asociaron significativamente con un nivel alto del IESF, con valores ORa: 1,17 (IC95%: 1,05-1,31) p=0,005 y ORa: 1,22 (IC95%: 1,09-1,49) p=0,044 respectivamente. Podemos concluir que la exposición ambiental al consumo, el consumo de alcohol y actuar con urgencia y la falta de perseverancia, se asociaron con nivel alto de susceptibilidad. La identificación estos factores permitirá orientar los programas preventivos mejorando los esfuerzos dirigidos a prevenir el inicio de consumo de tabaco.

The Expanded Susceptibility to Smoking Index (ESSI) is a marker of initiation to tobacco use that allow to identify young people who are at risk of starting to smoke. The aim of this study was to analyze the factors associated with IESF in non-smoking students of 2nd-4th of secondary education. A cross-sectional study was conducted in three secondary schools in Cáceres. Social and demographic variables, cigarette consumption (personal, family and social environment), alcohol and other substances consumption, intrapersonal factors (impulsivity and affect) and ESSI which was categorized in three levels. Of the 377 participants (47% boys, median age 15[14-15]), 226 (60%) had not tried cigarettes, of which 110 (48,7%) were classified as susceptible-highly susceptible. Multivariate analyses showed significant quantities of association between family consumption, alcohol consumption, having friends who smoke with the highly susceptible condition, with aOR values: 2.78 (95% CI: 1.17-6.51) p=0.020, aOR: 4.67 (95% CI: 1.74-12.5) p=0.002 and aOR: 3.85 (95%CI: 1.67-9.04) p=0.002. The urgency and lack of perseverance subscales of impulsivity were significantly associated with a high level of ESSI, with aOR values: 1.17 (95%CI: 1.05-1.31) p=0.005 and aOR: 1.22 (95%CI: 1.09-1.49) p=0.044 respectively. A higher ESSI score was associated with greater exposure to people smoking in the home, having friends who smoke, alcohol consumption, and a higher score in the scales of urgency and lack of perseverance of impulsivity. Identifying these factors will enable preventive program to be targeted by improving efforts to prevent the onset of tobacco use.
