dc.description.abstract | OBJETIVO: El objetivo de este estudio fue desarrollar un fantasma hepático multimodal y permanente que mostrara la vasculatura funcional y las patologías comunes, para la enseñanza, la formación y el desarrollo de equipos en la ecografía laparoscópica y la navegación. MÉTODOS: La cera fundida fue inyectada simultáneamente en las venas portal y hepática de un hígado humano. Al solidificarse la cera, el tejido hepático circundante se disolvió, dejando un molde de los vasos. Se estableció una conexión entre los dos árboles vasculares manipulando manualmente la cera. El molde se colocó, junto con diferentes modelos de tumores multimodales, en un molde con forma de hígado, que posteriormente se llenó con un polímero. Después de curarse, la cera se derritió y se eliminó del modelo, estableciendo así un sistema de canales interconectados que reproducían la vasculatura principal del hígado original. Así, un líquido puede circular a través del modelo de una manera que imita de cerca el flujo sanguíneo natural. RESULTADOS: Tanto los modelos de tumores, es decir, los tumores metastásicos, el carcinoma hepatocelular y el quiste benigno, como los vasos dentro del modelo de hígado, fueron visualizados claramente por las tres modalidades de imágenes: CT, MR y ultrasonido. Las imágenes de ultrasonido Doppler de los vasos demostraron la funcionalidad del flujo sanguíneo del fantasma. CONCLUSIÓN: Mediante un procedimiento de fundición en dos pasos, produjimos un fantasma hepático multimodal, con canales vasculares abiertos, y modelos de tumores, que es lo más parecido a la práctica de procedimientos de imagen y guía en animales o humanos. La técnica es en principio aplicable a cualquier órgano del cuerpo. | es_ES |
dc.description.abstract | PURPOSE: The objective of this study was to develop a multimodal, permanent liver phantom displaying functional vasculature and common pathologies, for teaching, training and equipment development in laparoscopic ultrasound and navigation. METHODS: Molten wax was injected simultaneously into the portal and hepatic veins of a human liver. Upon solidification of the wax, the surrounding liver tissue was dissolved, leaving a cast of the vessels. A connection was established between the two vascular trees by manually manipulating the wax. The cast was placed, along with different multimodal tumor models, in a liver shaped mold, which was subsequently filled with a polymer. After curing, thewaxwas melted and flushed out of the model, thereby establishing a system of interconnected channels, replicating the major vasculature of the original liver. Thus, a liquid can be circulated through the model in a way that closely mimics the natural blood flow. RESULTS: Both the tumor models, i.e., the metastatic tumors, hepatocellular carcinoma and benign cyst, and the vessels inside the liver model, were clearly visualized by all the three imaging modalities: CT, MR and ultrasound. Doppler ultrasound images of the vessels proved the blood flow functionality of the phantom. CONCLUSION: By a two-step casting procedure, we produced a multimodal liver phantom, with open vascular channels, and tumor models, that is the next best thing to practicing imaging and guidance procedures in animals or humans.
